INTRODUCTION:
Welcome to the Provide Personal Support to People with Individualised Plans course. This course only includes a challenge assessment designed to assess your knowledge and skills required to demonstrate competence in delivering safe, respectful and person-centred personal support that is guided by individualised plans and aligned with organisational policies and legal requirements.
Personal support in the context of individualised plans involves working in a person-centred and strengths-based way to uphold the person’s independence, choice and self-determination. It requires an understanding of the role and purpose of individualised plans in directing day-to-day support, and the ability to consult with the person, their family, carers and other key people to confirm support requirements and preferences. It also involves preparing appropriately for support by confirming, assembling and safely using the required equipment, aids and assistive technologies, and then providing safe, dignified assistance with personal care activities such as dressing, grooming, showering, toileting, eating, drinking and mobility.
The course assessment focuses on your ability to interpret and apply individualised plans in real support situations, and to recognise the boundaries and responsibilities of your role. You will be required to demonstrate how you follow organisational procedures when assisting a person to take pre-packaged medication within your role boundaries, and how you apply legal and ethical requirements in personal support, including privacy, confidentiality, duty of care, dignity of risk, human rights, anti-discrimination and mandatory reporting obligations. The assessment will also examine your understanding of how to maintain a safe, healthy and comfortable environment, including the application of infection prevention and control practices and the safe use of manual handling and transfer techniques such as slide sheets, hoists, slings, lifters, bed–chair and chair–standing transfers, and responding appropriately after a fall.
By successfully completing this course assessment, you will have demonstrated that you can interpret and apply the core requirements of providing personal support to people with individualised plans, including working in a person-centred and strengths-based way, preparing and delivering safe and respectful personal care, managing risks within your scope of role, collaborating with the person’s support team, and maintaining high standards of documentation and confidentiality—confirming your readiness to provide reliable, rights-based personal support in community services or care settings.
TOPICS COVERED IN THIS ASSESSMENT:
This course assessment is based on the following topics:
1. Role and purpose of individualised plans in guiding personal support.
2. Person-centred and strengths-based approaches to supporting independence, choice and self-determination.
3. Consulting with the person, family, carers and others to confirm personal support requirements and preferences.
4. Preparing for support: confirming, assembling and safely using required equipment, aids and assistive technologies.
5. Providing safe, respectful personal care (dressing, grooming, showering, toileting, eating, drinking and mobility support).
6. Assisting a person to take pre-packaged medication within role boundaries and organisational procedures.
7. Legal and ethical requirements in personal support (privacy, confidentiality, duty of care, dignity of risk, human rights, discrimination, mandatory reporting).
8. Maintaining a safe, healthy and comfortable environment, including infection prevention and control practices.
9. Safe manual handling and transfer techniques (slide sheets, hoists, slings, lifters, bed–chair and chair–standing transfers, falls recovery).
10. Monitoring support activities, recognising potential or actual risks, and responding within scope of role.
11. Identifying signs of additional or unmet needs and reporting or referring according to organisational procedures.
12. Working effectively with the person’s support team (family, carers, health professionals, supervisors and other support workers).
13. Supporting and reviewing the person’s use of assistive technologies across daily living activities.
14. Participating in discussions with the person and supervisor to review how support is meeting needs and confirm changes required.
15. Completing, maintaining and storing reports and documentation in line with organisational policies, while upholding confidentiality and privacy.
